Housekeeper Agreement

This document serves as a participation agreement between MAIDtoMATCH ("the Platform") and the undersigned service provider ("Provider").

Completion and signature of this document allows MAIDtoMATCH to evaluate and potentially match the Provider with households seeking services.

Participation Agreement

By signing this document, the Provider agrees to the following terms:

1. No Participation Fee

MAIDtoMATCH does not charge any fee to the Provider for participation on the placement service or for being introduced to potential clients.

2. 30 - Day Commitment

   When matched with a client, the provider agrees to commit to 30 days with that household after the initial 1 day trial. Providers who leave the placement before 30 days without good cause may be removed from the placement service without notice.

   
   

3. Privacy & Consent

   MAIDtoMATCH collects personal information and identification in order to verify identity and provide clients with suitable housekeepers. Idetification documents are stored securily and never shared with clients, and are deleted 30 days after the provider leaves the roster. By signing this agreement, the provider consents to the collection and use of this information for these purposes.

   
   

4. Accurate Information

The Provider certifies that all information supplied in this document and any supporting documentation is true, accurate, and authentic to the best of their knowledge.

Any false or misleading information may result in removal from the MAIDtoMATCH placement service.

3. Documentation Verification

The Provider agrees to provide supporting documentation upon request, including identification and other relevant credentials, and confirms that all documents submitted are valid and authentic.

4. Updates and Communication

The Provider agrees to notify MAIDtoMATCH of relevant updates, including but not limited to:

• Changes to contact information

• Changes in availability

• Changes in service offerings

• Changes in location or service areas

5. Work Engagement Updates

To maintain accurate matching and service records, the Provider agrees to inform MAIDtoMATCH of any relevant changes in client engagements, including:

• New clients obtained through MAIDtoMATCH

• New clients obtained independently

• Termination of client relationships

• Situations where a client relationship ended due to resignation or dismissal

This information is used solely for internal matching and service management purposes.

6. Independent Contractor Status

The Provider acknowledges that:

• MAIDtoMATCH acts solely as a matching placement service between households and service providers.

• The Provider operates as an independent service provider.

• MAIDtoMATCH does not employ the Provider and is not responsible for wages, taxes, insurance, or employment benefits.

7. Removal from Platform

MAIDtoMATCH reserves the right to remove any Provider from the placement service at its discretion, including but not limited to situations involving:

• Misrepresentation of information

• Client complaints

• Failure to comply with the participation terms

Limitation of Liability

The Provider acknowledges and agrees that MAIDtoMATCH acts solely as a matching and referral platform connecting independent service providers with households seeking services.

MAIDtoMATCH does not employ, supervise, manage, or control the work performed by Providers and is not a party to any agreement between the Provider and any client.

Accordingly, the Provider agrees that:

Independent Relationship

All work arrangements, schedules, compensation, and services are negotiated and performed directly between the Provider and the client.

No Responsibility for Work Performed

MAIDtoMATCH shall not be responsible or liable for any work performed by the Provider, including but not limited to:

• service quality

• property damage

• personal injury

• disputes between Provider and client

• unpaid services or payment disagreements

• termination of services

Assumption of Risk

The Provider acknowledges that they perform services at their own discretion and assume all risks associated with providing services to clients.

Responsibility for Theft or Damage

Providers are personally responsible for theft or damage to client property. MAIDtoMATCH will cooperate fully with law enforcement and clients. Any provider found resonsible will be removed from the placement service immediately.

Limitation of Liability

To the maximum extent permitted by applicable law, MAIDtoMATCH, its owners, operators, and affiliates shall not be liable for any direct, indirect, incidental, consequential, or special damages arising from or related to:

• introductions made through the platform

• services provided by the Provider

• interactions between Providers and clients

Provider Responsibility

The Provider remains solely responsible for:

• their conduct

• their services

• their safety

• their compliance with applicable laws and regulations
